Hospitals face a genuine dilemma. The operational benefits of AI are clear: faster document processing, automatic coding, improved searchability of patient records. Yet most AI platforms require data to leave the organisation and travel to external servers. For medical institutions, that is not an acceptable trade-off. Patient data is governed by strict regulations, including GDPR and sector-specific standards such as HL7. Any transfer outside the organisation creates legal exposure and reputational risk.

Large language models (LLMs) or smaller specialised models (SLMs) are installed on-premises, on the hospital's own servers. No patient record ever leaves the institution. Sensitive data remains on-premises, while less critical processing tasks can run in a private cloud environment if needed. It is also possible to fine-tune models on hospital-specific data. A model trained on the institution's own discharge letters, laboratory reports, or nursing notes performs significantly better than a generic equivalent. The fine-tuning process runs entirely in-house: training data never leaves the organisation, and the resulting model remains the property of the hospital.
